Unfortunately, we are unable to take any new patients currently. We may be able to consider family members of those that are already enrolled at the practice. You are welcome to email admin@ochc.co.nz and ask if we can enrol you, (if you are related to a family member). For more information, please download the enrolment guide.
Please print off, complete and sign our enrolment form and return it to the Health Centre. You can either post it or bring it to us at the clinic.
